(b) Print an application (To print, click on the printer icon within Adobe Acrobat Reader, do not utilize "File," "Print" within you browser)
(c) Fill out the application
(d) Mail application along with pre-entry fee of $20 (fee must be in the form of a check, made payable to "United Way") to:
